Cagrilintide: A Comprehensive Guide to Uses, Research, Dosing, Forms, and Potential Benefits
Cagrilintide is one of the more interesting investigational peptides currently being developed for obesity, appetite regulation, weight management, and metabolic health.
Unlike semaglutide, tirzepatide, and retatrutide, cagrilintide is not primarily a GLP-1-based compound. Instead, it is a long-acting amylin analogue, meaning it is designed to mimic and enhance some of the biological effects of the naturally occurring hormone amylin.
Amylin is normally released from pancreatic beta cells alongside insulin after a meal. Among its functions, it contributes to satiety, influences gastric emptying, and helps regulate how much food a person consumes.
That makes the amylin pathway an attractive target for obesity treatment.
Cagrilintide has already produced meaningful weight loss in clinical research as a once-weekly treatment, and Novo Nordisk is now developing cagrilintide as a standalone medication through its Phase 3 RENEW clinical program. A subanalysis of the Phase 3 REDEFINE 1 study found that participants receiving cagrilintide 2.4 mg lost an average of approximately 11.8% of their body weight after 68 weeks, compared with 2.3% for placebo among participants who remained on treatment.
Cagrilintide is also a key component of CagriSema, an investigational combination of cagrilintide and semaglutide that Novo Nordisk has submitted to the FDA for approval.
Despite that progress, cagrilintide itself remains investigational and is not currently an FDA-approved medication.
What Is Cagrilintide?
Cagrilintide is a synthetic, long-acting analogue of amylin.
Amylin is a peptide hormone consisting of 37 amino acids. It is produced by pancreatic beta cells and normally released at the same time as insulin in response to food.
While insulin is best known for regulating blood glucose, amylin performs several complementary functions.
It helps signal that a meal has been consumed and contributes to:
- Satiety
- Appetite regulation
- Meal-size reduction
- Slower gastric emptying
- Regulation of glucagon secretion after eating
- Overall energy intake
Because of these effects, scientists have been interested in amylin-based obesity treatments for many years.
Cagrilintide was engineered to provide a much longer duration of action than natural amylin, allowing it to be investigated as a once-weekly injection.
What Does Amylin Do?
To understand cagrilintide, it helps to understand how amylin fits into normal eating behavior.
When food enters the gastrointestinal system, numerous hormones communicate with the brain.
These signals essentially tell the brain:
Food is arriving.
Nutrients are becoming available.
You are becoming full.
Amylin participates in this process.
One of its primary effects involves the brain regions responsible for appetite and satiety. Amylin signaling can reduce the desire to continue eating and help limit meal size.

This mechanism is one reason researchers believe enhancing amylin signaling may help people with obesity reduce overall calorie intake.
How Does Cagrilintide Work?
Cagrilintide activates amylin receptors.
These receptors are found in several areas of the nervous system, including brain regions involved in appetite and metabolic regulation.
Unlike stimulant-based appetite suppressants, cagrilintide does not simply increase sympathetic nervous system activity.
Instead, it attempts to strengthen the body’s own physiological satiety signaling.
The result may include:
reduced hunger
earlier fullness during meals
smaller portion sizes
reduced calorie consumption
and eventually weight loss.
This makes cagrilintide mechanistically distinct from GLP-1 receptor agonists.
Cagrilintide Versus GLP-1 Medications

This combination has produced substantial weight loss in Phase 3 clinical research.
In the REDEFINE 1 Phase 3 trial, participants with obesity or overweight and related health conditions who adhered to treatment lost an average of approximately 22.7% of their body weight after 68 weeks, compared with approximately 2.3% with placebo. More than 40% achieved weight loss of at least 25%.
Based on the Phase 3 program, Novo Nordisk submitted CagriSema to the FDA in December 2025 for chronic weight management.
That application concerns the combination treatment, not approval of standalone cagrilintide.
What Do People Use Cagrilintide For?
Since cagrilintide remains investigational, it is more accurate to discuss research applications and areas of interest rather than established medical uses.
Weight Loss
This is the primary reason for cagrilintide’s development.
In the cagrilintide-only arm of REDEFINE 1, participants receiving 2.4 mg once weekly lost an average of approximately 11.8% of body weight at 68 weeks under the treatment-adherence analysis.
That is significant weight loss for a drug that does not rely on GLP-1 receptor activation.
More than 30% of participants receiving cagrilintide alone achieved at least 15% weight loss in that analysis.
These findings encouraged Novo Nordisk to begin dedicated Phase 3 development of cagrilintide as a standalone obesity treatment.
Appetite Reduction
Because amylin signaling influences satiety, cagrilintide is being studied extensively for its effects on appetite.
A dedicated clinical study launched in 2026 is specifically investigating how cagrilintide influences food intake and appetite in adults with overweight or obesity.
Researchers are interested in whether cagrilintide reduces:
hunger between meals
portion size
food cravings
overall calorie consumption
and potentially the mental drive to continue eating after physiological energy needs have been met.
Obesity
Novo Nordisk is now developing cagrilintide specifically as a standalone obesity therapy through the RENEW Phase 3 program.
RENEW 1 is evaluating cagrilintide versus placebo in adults with obesity or overweight. The study began in November 2025 and is expected to complete during 2027.
This is important because it moves cagrilintide beyond simply being a component of CagriSema.
Type 2 Diabetes and Obesity
Cagrilintide is also being studied in people who have obesity or overweight along with type 2 diabetes.
The Phase 3 RENEW 2 trial is evaluating weight loss and safety in this population. The study began in November 2025 and is expected to continue through 2027.
As with other weight-management therapies, improvements in body weight can potentially influence blood glucose, insulin sensitivity, cardiovascular risk factors, and other metabolic measures.
Cagrilintide as a Standalone Weight-Loss Drug
For several years, much of the attention surrounding cagrilintide focused on CagriSema.
That began to change as researchers analyzed cagrilintide’s independent effects.
In REDEFINE 1, approximately 302 participants received cagrilintide 2.4 mg alone.
Among people adhering to treatment, average weight reduction was approximately 11.8% after 68 weeks, compared with 2.3% with placebo.
That is below the roughly 22.7% observed with CagriSema under similar analysis, but it remains clinically meaningful.
The results were strong enough for Novo Nordisk to launch the dedicated RENEW Phase 3 program.
That raises an interesting possibility: cagrilintide could eventually provide an alternative approach for some people who either do not require or do not tolerate GLP-1-based treatment.
Whether that becomes a real-world clinical advantage will depend on Phase 3 results and regulatory review.
Cagrilintide Dosing
There is currently no FDA-approved cagrilintide dosing schedule because cagrilintide itself remains investigational.
Doses discussed in this article therefore describe controlled clinical research rather than recommendations for personal use.
The dose receiving the greatest attention in late-stage research is:
2.4 mg once weekly by subcutaneous injection.
This was the cagrilintide maintenance dose evaluated in the REDEFINE 1 analysis and the dose used as the cagrilintide component of CagriSema.

The clinical formulation currently under development is administered by subcutaneous injection once weekly.
This means the medication is delivered into fatty tissue beneath the skin.
Cagrilintide was specifically engineered as a long-acting amylin analogue so that frequent daily administration would not be required.
ClinicalTrials.gov currently lists several studies involving subcutaneous cagrilintide, including Phase 3 obesity trials and pharmacokinetic studies comparing different versions of the compound.
What Forms Is Cagrilintide Offered In?
This subject requires an important distinction between legitimate investigational cagrilintide and products advertised through research suppliers.
Investigational Injection
The established clinical-research form is a long-acting subcutaneous injectable preparation developed by Novo Nordisk.
This is the formulation being evaluated in regulated human clinical trials.
CagriSema Combination Injection
Cagrilintide is also being developed as part of CagriSema, containing cagrilintide 2.4 mg plus semaglutide 2.4 mg.
Novo Nordisk submitted this combination to the FDA for approval in December 2025.
Lyophilized Research Powder
Some research-material suppliers advertise cagrilintide as lyophilized, or freeze-dried, powder.
Lyophilization is commonly used with research peptides because removing water can improve storage stability.
However, a vial labeled “cagrilintide” from an online research supplier is not the same thing as Novo Nordisk’s investigational pharmaceutical formulation.
Such material should not automatically be assumed to have equivalent:
purity
concentration
sterility
stability
or pharmacokinetics.
Research Vials
Cagrilintide may also appear in research vials containing various milligram quantities.
The presence of a peptide in a sealed vial does not establish that it is approved or suitable for human injection.
There is currently no FDA-approved retail cagrilintide vial.
Common Side Effects in Clinical Research
Because cagrilintide influences satiety and gastrointestinal function, many of the commonly reported adverse events have involved the digestive system.
In the cagrilintide-only arm of REDEFINE 1, gastrointestinal adverse events occurred in approximately 54.6% of cagrilintide-treated participants, compared with about 40.7% receiving placebo.
Reported gastrointestinal events included approximately:
Nausea: 23.8%
Constipation: 20.9%
Diarrhea: 15.6%
Vomiting: 7.0%
Despite these events, only about 1.3% of participants discontinued cagrilintide because of gastrointestinal adverse events in that analysis.
This has generated interest in whether cagrilintide might eventually provide a favorable balance between weight loss and gastrointestinal tolerability for some patients.
Larger standalone Phase 3 studies will provide more reliable safety information.
Cagrilintide and Muscle Loss
Any treatment producing significant weight loss raises questions about lean body mass.
When people lose a substantial amount of weight, some of that reduction commonly comes from:
body fat
and some from
lean tissue.
This can occur during conventional calorie restriction, treatment with obesity medications, or following bariatric surgery.
For that reason, preserving muscle during weight loss is increasingly considered an important part of obesity treatment.
Adequate protein intake and resistance training are commonly used strategies during medically supervised weight reduction.
Future cagrilintide studies should provide a clearer understanding of how its weight loss is distributed between fat mass and lean tissue.
Cagrilintide Versus Semaglutide
Cagrilintide and semaglutide both reduce food intake but use different mechanisms.
Cagrilintide: amylin receptor activity.
Semaglutide: GLP-1 receptor activity.
Semaglutide has a much larger clinical evidence base and is already FDA-approved for several indications.
Cagrilintide remains investigational.
The real scientific excitement comes from the fact that the two mechanisms appear to complement one another.
That is why CagriSema produced considerably greater weight reduction than cagrilintide alone in REDEFINE 1.
Cagrilintide Versus Tirzepatide
Tirzepatide activates GIP and GLP-1 receptors, while cagrilintide primarily targets the amylin pathway.
Head-to-head research has focused primarily on CagriSema versus tirzepatide, rather than cagrilintide alone.
In the REDEFINE 4 Phase 3 study announced in February 2026, participants receiving CagriSema lost approximately 23.0% of body weight after 84 weeks when evaluating those who remained on treatment, compared with approximately 25.5% with tirzepatide 15 mg.
CagriSema therefore failed to demonstrate the trial’s predefined statistical criterion for non-inferiority to tirzepatide.
That result illustrates an important lesson: combining additional hormone pathways does not automatically guarantee superior clinical weight loss.
Is Cagrilintide FDA Approved?
No.
As of August 2026, cagrilintide remains an investigational medicine.
Novo Nordisk’s Phase 3 RENEW studies explicitly describe cagrilintide as a new investigational medicine that doctors cannot yet prescribe.
However, the related combination CagriSema is further along.
Novo Nordisk submitted an FDA New Drug Application for CagriSema in December 2025 for chronic weight management in adults with obesity or overweight plus at least one weight-related condition.
An FDA submission does not mean a drug has been approved.
FDA must still evaluate the treatment’s effectiveness, safety, manufacturing processes, labeling, and overall benefit-risk profile.
Current Research in 2026
Cagrilintide research has accelerated substantially.
The compound is now being developed through multiple clinical programs.
The Phase 3 RENEW 1 study is evaluating standalone cagrilintide for adults with obesity or overweight and is expected to continue into 2027.
RENEW 2 is conducting a similar investigation among participants who also have type 2 diabetes.
Researchers are additionally studying exactly how cagrilintide changes food intake and subjective appetite, rather than simply measuring changes on the scale.
Pharmacokinetic research is also underway comparing different formulations of cagrilintide in people with obesity or overweight.
Meanwhile, CagriSema continues through an extensive Phase 3 program evaluating obesity, diabetes, cardiovascular outcomes, eating behavior, and related metabolic conditions.
What Researchers Still Need to Learn
Despite impressive progress, several important questions remain.
Researchers need additional information regarding:
long-term weight maintenance
cardiovascular outcomes
long-term gastrointestinal tolerability
effects on lean body mass
optimal dose escalation
which patients respond best
how cagrilintide compares directly with GLP-1 therapies
and whether standalone cagrilintide offers advantages for people who cannot tolerate GLP-1 medications.
The ongoing Phase 3 RENEW program should provide considerably more information over the next several years.
The Bottom Line
Cagrilintide represents an important new approach to modern obesity research.

There is therefore no FDA-approved dosing regimen, no approved retail cagrilintide product, and no established protocol for personal use.
Clinical research has primarily investigated once-weekly subcutaneous administration, with 2.4 mg being the maintenance dose receiving the greatest attention in late-stage research.
Perhaps the most accurate way to describe cagrilintide in 2026 is as a promising next-generation amylin-based weight-management peptide with meaningful late-stage human clinical evidence, but one that is still undergoing the research necessary to establish its ultimate role, approved dosing, long-term safety, and appropriate medical use.
What makes cagrilintide particularly interesting is that it demonstrates something increasingly important in obesity medicine: GLP-1 is not the only hormone pathway capable of producing significant changes in appetite and body weight.
Amylin represents another powerful component of the body’s natural system for controlling food intake.
Whether cagrilintide ultimately becomes an important standalone obesity medication, is used primarily alongside GLP-1 therapies, or serves both roles will become clearer as the ongoing Phase 3 program produces additional results.
